The aim of the study was to investigate the effects of several decalcifying agents used as irrigant solutions in endodontic treatment on collagen and mineral components of dentin. Coronal dentin discs from five caries-free human third molars with a smear layer were treated for one minute with a chelating solution (1% Ethylenediaminetetraacetic acid (EDTA), 10% EDTA, 17% EDTA, 10% citric acid). Mineralization degree (Ca/N and P/N atomic ratios, IR Iapatite/Iamide II and I1410(carbonate)/I554(phosphate) spectroscopic ratios) and possible collagen rearrangements (collagen infrared (IR) amide II e III shifts) were evaluated by environmental scanning electron microscopy (ESEM)/energy dispersive X-ray spectroscopy (EDX) and IR spectroscopy before and after treatment (T0) and after ageing (T24h and T2m) in simulated body fluid (SBF). At T0, analysis showed that the highest demineralizing effect was achieved using a 10% citric acid solution and 10% EDTA, while the smallest effect was observed when using 17% EDTA. No significant collagen modifications were detected upon treatment with 1% EDTA, while subtle changes were observed after the other treatments. At T24h or T2m, analyses showed the highest remineralization values for 1% EDTA and the lowest for 10% citric acid, mainly at T2m. The samples treated with 17% EDTA showed slight collagen rearrangements upon remineralization. In conclusion, the highest demineralizing effect was observed for 10% EDTA and 10% citric acid. Collagen rearrangement was found for all the treatments except for 1% EDTA. The highest remineralization capability in SBF values was recorded for 1% EDTA and the lowest for 10% citric acid. A slight collagen rearrangement upon remineralization was still present in 17% EDTA-treated samples. Clinical use as a chelating agent in the endodontic therapy of citric acid and concentrated EDTA solutions should be reconsidered.

© 2019 Elsevier B.V. Brain delivery of drugs by nanoparticles is a promising strategy that could open up new possibilities for the chemotherapy of brain tumors. As demonstrated in previous studies, the loading of doxorubicin in poly(lactide-co-glycolide) nanoparticles coated with poloxamer 188 (Dox-PLGA) enabled the brain delivery of this cytostatic that normally cannot penetrate across the blood-brain barrier in free form. The Dox-PLGA nanoparticles produced a very considerable anti-tumor effect against the intracranial 101.8 glioblastoma in rats, thus representing a promising candidate for the chemotherapy of brain tumors that warrants clinical evaluation. The objective of the present study, therefore, was the optimization of the Dox-PLGA formulation and the development of a pilot scale manufacturing process. Optimization of the preparation procedure involved the alteration of the technological parameters such as replacement of the particle stabilizer PVA 30–70 kDa with a presumably safer low molecular mass PVA 9–10 kDa as well as the modification of the external emulsion medium and the homogenization conditions. The optimized procedure enabled an increase of the encapsulation efficiency from 66% to >90% and reduction of the nanoparticle size from 250 nm to 110 nm thus enabling the sterilization by membrane filtration. The pilot scale process was characterized by an excellent reproducibility with very low inter-batch variations. The in vitro hematotoxicity of the nanoparticles was negligible at therapeutically relevant concentrations. The anti-tumor efficacy of the optimized formulation and the ability of the nanoparticles to penetrate into the intracranial tumor and normal brain tissue were confirmed by in vivo experiments.

© 2019 The essence of our hypothesis is quantum properties of conscious processes as well as the possibility to measure changes in conscious attention by using quantum double-slit experiment. We supposed that the act of observing in state of focused attention cause a wave function collapse in double-slit diffraction. In order to test the hypothesis, 26 participants took part in the study divided into physical and physiological parts of the experiment. The purpose of the physical system was to reproduce the brain quantum process via hypothesized quantum entanglement. The physical part consisted of a red laser source, neutral density filter, double-slit diaphragm, and linear couple charged camera, while the physiological part of the experiment was significant for the physiological quantifying state of attention. Physiological data were collected by using 29 channel electrophysiological unit with 21 channel electroencephalograph. The study had control and experimental group according to dependent variables measured in the physical part of the experiment. The data in the experimental group were collected over ten studies (sessions). Results obtained in hypothesis testing showed significant increases in corpuscular properties of the electromagnetic wave as well as significant quantum entanglement between the brain and external double-slit quantum system. Our results also offer insight into the connection between the chaotic dynamic of the electroencephalographic signals and uncertainty in the physical system due to focused attention effect. We also hypothesized that the state of concentrated attention was highest during the first several seconds. The last hypothesis considered possible backward time referral effect of cognitive evoked potential p300.

Water solubility of prothionamide and its solid dispersions with polyethylene glycol-1500, polyvinylpyrrolidone-10000, and β-cyclodextrin was studied. The most promising results were obtained with the solid dispersion with polyvinylpyrrolidone, which provided a 2.6-fold general increase of drug’s solubility and about a 6-fold dissolution rate increase, the maximal values among the studied drug forms. The results of complex physicochemical studies suggested that the improved drug release was due to micronization and solubilizing action of the polymer. A principal factor was the destruction of the crystal lattice and formation of amorphous particles in the solid dispersion. The obtained data can be useful for the development of modified pharmaceutical forms of prothionamide with improved drug release profile.

To analyze the effect of gravity on the structure of germinal tissues, we examined tissues of the testes and duct deferens of mice that were exposed to space flight conditions for 21–24 days (experiment Rodent Research-4, SpaceX-10 mission, February 2017, USA). We evaluated the levels of cytoskeletal proteins, sperm-specific proteins, and epigenetic events; in particular, we evaluated levels of 5-hydroxymethylcytosine and of enzymes that regulate DNA methylation/demethylation. We did not detect changes in the levels of cytoskeletal proteins, sperm-specific proteins, DNA-methylases, DNA demethylases, DNA acetylases, or histone deacetylases. However, there were changes at the gene expression level. In particular, there was an increase in the demethylase Tet2 and a decrease in the histone deacetylase Hdac1. These gene expression changes may be of key importance during the early period of readaptation since they could lead to an increase in the expression of target genes.

© 2019 The Authors Sylvian fissure arachnoid cysts are known intracranial lesion in pediatric population. We describe a case of symptomatic cyst of Sylvian fissure that was managed with a standard endoscopic cystocisternostomy and re-endoscopy. We also describe the techniques of fenestration of medial wall cyst and partial removal of arachnoid membrane over third cranial nerve in anterior part (region near Dolenc medial triangle) and posterior part (region near Fukushima superior (paramedial) triangle) prevent closure of fenestration and recurrence of hypertension. A 6 years old girl presented with progressively increasing headache, left side hemiparesis, drag resistant of symptomatic epilepsy. MRI and CT scan showed right Sylvian fissure arachnoid cyst. The first endoscopic fenestration of cyst was done on standard basis. After operation patient improve clinically and radiologically. Two years later the patient came back with recurrent symptoms. The operation was performed by prof. Sufianov A. Endoscopic reoperation showed closure of fenestration in arachnoid membrane. Endoscopic re-fenestration with partial excision of arachnoid on over third cranial nerve in anterior and posterior part was excised. The patient again recovered radiologically and clinically till last follow up. In Sylvian fissure arachnoid cysts, endoscopic treatment with wide fenestration of medial wall of cyst preferably with partial removal of arachnoid may be very useful and preventing recurrent symptoms.

© 2019, The Author(s). Under microgravity, the gene expression levels vary in different types of cells; however, the reasons for this have not been sufficiently studied. The aim of this work was to evaluate the methylation of CpG islands in the promoter regions of the genes encoding some cytoskeletal proteins, the total methylation and 5 hmC levels, and the levels of enzymes that regulate these processes in the testes, heart, and lungs in mice after a 30-day microgravity modeling by antiorthostatic suspension and after a subsequent 12-hour recovery as well as in the corresponding control group and identical groups treated with essential phospholipids. The obtained results indicate that under modeling microgravity in the examined tissues a decrease of cytoskeletal gene expression (mainly in the heart and lungs tissues) correlated with an increase in the CpG islands methylation and an increase of the expression (mainly in the testes tissue) – with a decrease of the CpG-methylation, despite of the fact that in the examined tissues took place a decrease of the content methylases and demethylases. But the deacetylase HDAC1 content increased in the heart and lungs tissues and decreased in the testes, letting us suggest its participation in the regulation of the methylation level under microgravity conditions.

Background: Sudden cardiac death (SCD) refers to unanticipated death as a consequence of cardiac failure which takes place over a brief time period (typically within an hour of the commencement of symptoms) in an individual, who may or may not be known to be subject to cardiac disease, but in whom no fatal condition has been diagnosed. The objective of this investigation was to identify the potential cause of SCD in the young population in Moscow. Result: Analysis revealed that the mean age of the deceased amongst men was 24.6 years, and amongst women was 29.1 years. Seventy-six percent of sudden deaths were found to result from some previously undiagnosed pathology of the cardiovascular system, and cardiomyopathy is the most common cause of death in young people. Coronary heart disease is the most common cause of death in people above 40 years old. Conclusion: Investigation to diagnose the causes of SCD constitutes a relevant, comprehensive, and multidisciplinary undertaking, which ought to be founded upon the application of novel and current diagnostic methodologies. An integrated strategy to the quest for the underlying reason for sudden death ought to be founded upon the outcomes of biochemical, anatomical, genetic, pathophysiological, and other investigations. The search and development of objective risk factors will permit timely therapeutic intervention for patients with cardiovascular pathology to minimise and prevent the risk of sudden cardiac death.

In the context of a post-antibiotic era, the phenomenon of microbial allolysis, which is defined as the partial killing of bacterial population induced by other cells of the same species, may take on greater significance. This phenomenon was revealed in some bacterial species such as Streptococcus pneumoniae and Bacillus subtilis, and has been suspected to occur in some other species or genera, such as enterococci. The mechanisms of this phenomenon, as well as its role in the life of microbial populations still form part of ongoing research. Herein, we describe recent developments in allolysis in the context of its practical benefits as a form of cell death that may give rise to developing new strategies for manipulating the life and death of bacterial communities. We highlight how such findings may be viewed with importance and potential within the fields of medicine, biotechnology, and pharmacology.

Patients with bipolar disorder (BD) show higher frequency of obesity and type 2 diabetes (T2D), but the underlying genetic determinants and molecular pathways are not well studied. Using large publicly available datasets, we (1) conducted a gene-based analysis using MAGMA to identify genes associated with BD and body mass index (BMI) or T2D and investigated their functional enrichment; and (2) performed two meta-analyses between BD and BMI, as well as BD and T2D using Metasoft. Target druggability was assessed using the Drug Gene Interaction Database (DGIdb). We identified 518 and 390 genes significantly associated with BD and BMI or BD and T2D, respectively. A total of 52 and 12 genes, respectively, were significant after multiple testing correction. Pathway analyses conducted on nominally significant targets showed that genes associated with BD and BMI were enriched for the Neuronal cell body Gene Ontology (GO) term (p = 1.0E−04; false discovery rate (FDR) = 0.025) and different pathways, including the Signaling by Hedgehog pathway (p = 4.8E−05, FDR = 0.02), while genes associated with BD and T2D showed no specific enrichment. The meta-analysis between BD and BMI identified 64 relevant single nucleotide polymorphisms (SNPs). While the majority of these were located in intergenic regions or in a locus on chromosome 16 near and in the NPIPL1 and SH2B1 genes (best SNP: rs4788101, p = 2.1E−24), five were located in the ETV5 gene (best SNP: rs1516725, p = 1E−24), which was previously associated with both BD and obesity, and one in the RPGRIP1L gene (rs1477199, p = 5.7E−09), which was also included in the Signaling by Hedgehog pathway. The meta-analysis between BD and T2D identified six significant SNPs, three of which were located in ALAS1 (best SNP: rs352165, p = 3.4E−08). Thirteen SNPs associated with BD and BMI, and one with BD and T2D, were located in genes which are part of the druggable genome. Our results support the hypothesis of shared genetic determinants between BD and BMI and point to genes involved in Hedgehog signaling as promising targets.

Here, I present the data testifying that the conversion of free radical NO molecules to nitrosonium ions (NO+), which are necessary for the realization of one of NO biological effects (S-nitrosation), may occur in living organisms after binding NO molecules to loosely bound iron (Fe2+ ions) with the subsequent mutual one-electron oxidation–reduction of NO molecules (their disproportionation). Inclusion of thiol-containing substances as iron ligands into this process prevents hydrolysis of NO+ ions bound to iron thus providing the formation of stable dinitrosyl iron complexes (DNIC) with thiol ligands. Such complexes act in living organisms as donors of NO and NO+, providing stabilization and transfer of these agents via the autocrine and paracrine pathways. Without loosely bound iron (labile iron pool) and thiols participating in the DNIC formation, NO functioning as one of universal regulators of diverse metabolic processes would be impossible.

© 2019 Elsevier Ltd The effect of different initiators (tert-butyl chloride (t-BuCl) and 2-chloro-2-phenylpropane (CumCl) on the cationic polymerization of isobutylene co-initiated by acidic chloroferrate imidazole-based ionic liquid emimCl-FeCl3 (emimCl: 1-ethyl-3-methylimidazolium chloride, molar fraction of FeCl3 (χ(FeCl3)) ≥ 0.6) in the presence of diisopropyl ether at 0 °C and [IB] = 5.2 M has been investigated. Generally, the use of all above-mentioned initiators results in increase of the monomer conversion from 30% to 60% as well as in decrease of molecular weight from 2500 g mol−1 to 1100 g mol−1 and polydispersity from 2.6 to 1.7, respectively, but does not influence the content of exo-olefin end group. It was demonstrated that CumCl does not initiated directly the polymerization at 0 °C or −20 °C, rather, it decomposed with the formation of the proton, a true initiator of the polymerization. It was also found that addition of small amounts (1% by volume) of benzene or its derivatives into the polymerization system leads to further increase of the monomer conversion (>90%). The rate of isobutylene polymerization depends on basicity of aromatic compounds added and increases in the following order: benzene < toluene ≈ mesitylene.

Purpose: An anterior glottic web consists of the formation of a bridge of scar tissue covered by epithelium between the anterior free edges of the true vocal cords and represents one of the most common complications of laryngeal endoscopic surgery for tumors involving the anterior commissure. Endoscopic surgery is the therapy of choice, but simple section of the web is burdened by a high recurrence rate. Topical application of mitomycin C, intracordal stents, and the use of mucosal microflaps have been proposed to improve outcomes. We report our experience with the use of single and double mucosal microflaps (sliding technique) during the management of iatrogenic anterior glottic web (IAGW). Methods: From November 2010 to December 2018, 30 patients (29 males, 1 female, mean age 65 years, range 47–87 years) were observed for IAGW, and 11 of these patients (36.7%) required surgical treatment. The Voice Handicap Index (VHI) and the GRBAS were used for the perceptive evaluation of pre- and post-operative voice quality. Results: A reduction of the web length was observed in all cases, and we did not observe any residual web at the mid-third of the glottis. The mean post-operative VHI score decreased from 45 to 24, and the mean post-operative GRBAS values were reduced from 2.8, 2.4, 2.3, 2.1, and 1.1 to 1.9, 1.4, 1.3, 1.1, and 0, respectively. Conclusions: The microflap technique represents an effective and reproducible one-step procedure that, in expert hands, allows to obtain good anatomical and functional results in a high percentage of cases.

© 2019 Objective: According to one of the hypotheses, somatoform disorders (SD) are related to disturbed perception of bodily signals. We suspect that the disturbances and abnormalities of multisensory integration dynamics may be an additional SD mechanism. The objective of the research was to investigate multisensory bodily illusion dynamics in SD patients. Methods: A clinical group of SD patients (n = 16) and a control group (n = 17) participated in experiments aimed at eliciting a visual-tactile rubber hand illusion (RHI) and a visual-kinesthetic virtual hand illusion (VHI). Results: For both illusions studied, the illusion dynamics in SD patients differed from those of healthy subjects. The visual-tactile illusion on the subjective level appeared to be less strong in SD patients, and no proprioceptive drift (PD) was detected. The subjective dynamics of the visual-kinesthetic illusion in patients were similar to those of the control group, but the PD dynamics were different: after the termination of stimuli from the artificial limb, the proprioceptive system did not return to its initial state; on the contrary, PD continued to increase. Conclusion: In comparison with the norm, patients with somatoform disorders display differences in multisensory mechanisms underlying the development of body perception. The results suggest that the disturbance of visual-tactile integration of the stimuli represents one of the mechanisms of body perception distortion in somatoform disorders. It may be assumed that one of the reasons for persistent symptom perception in SD patients is the rigidity of multisensory stimuli perception, at least in the visual-motor domain.

© 2019 Elsevier B.V. This study aimed at evaluating the remineralization effect promoted by different bioactive fillers on simulated early caries lesions in enamel (ECLE). Forty sound bovine incisors were used to prepare buccal enamel specimens (6 × 6 × 2 mm). The specimens were divided into two areas (3 × 3 mm²): control (CTR) and experimental (EXP). All the enamel specimens were then submitted to a specific protocol for a period of 48 h to induce simulated caries lesions. Subsequently, the specimens were treated for 7 days (2 min, twice a day) with a slurry pastes containing different bioactive particles (P/L ratio: 1 g/mL). All the specimens were analyzed by laser speckle before and after treatments. The results showed after the first analysis that the ECLE had very low average intensity (back-scattered light). Conversely, after application of the bioactive pastes, higher average intensity was always detected; this was comparable to CTR sound specimens. In conclusion, innovative pastes/gels developed for enamel remineralization should contain bioactive particles that when applied daily on early caries lesions may “boost” the remineralization process to reestablish a sound enamel.

Pathologic response and surgical outcomes in patients undergoing nephrectomy following receipt of immune checkpoint inhibitors for renal cell carcinoma

© 2019 Elsevier Inc. Objective: To evaluate the pathologic response, safety, and feasibility of nephrectomy following receipt of immune checkpoint inhibition (ICI) for renal cell carcinoma (RCC). Methods: Patients who underwent nephrectomy for RCC after exposure to nivolumab monotherapy or combination ipilimumab/nivolumab were reviewed. Primary surgical outcomes included operative time (OT), estimated blood loss (EBL), length of stay (LOS), readmission rates, and complication rates. Pathologic response in the primary and metastatic sites constituted secondary outcomes. Results: Eleven nephrectomies (10 radical, 1 partial) were performed in 10 patients after ICI with median postoperative follow-up 180 days. Six patients received 1 to 4 cycles of ipilimumab/nivolumab, while 5 received 2 to 12 infusions of nivolumab preoperatively. Five surgeries were performed laparoscopically, and 4 patients underwent concomitant thrombectomy. One patient exhibited complete response (pT0) to ICI, and 3/4 patients who underwent metastasectomy for hepatic, pulmonary, or adrenal lesions exhibited no detectable malignancy in any of the metastases resected. No patients experienced any major intraoperative complications, and all surgical margins were negative. Median OT, EBL, and LOS were 180 minutes, 100 ml, and 4 days, respectively. Four patients experienced a complication, including 3 that were addressed with interventional radiology procedures. One patient died of progressive disease >3 months after surgery, and 1 patient succumbed to pulmonary embolism complicated by sepsis. No complications or readmissions were noted in 6 patients. Conclusion: Nephrectomy following ICI for RCC is safe and technically feasible with favorable surgical outcomes and pathologic response. Timing of the nephrectomy relative to checkpoint dosing did not seem to impact outcome. Biopsies of lesions responding radiographically to ICI may warrant attention prior to surgical excision.

© 2019 Elsevier B.V. Submicrocapsules were prepared from diethylaminoethyl dextran (DEAE-D), xanthan gum (XG) and bovine serum albumin (BSA) on oil cores by ultrasonic treatment. These capsules were modified with poly-L-lysine (PLL) via electrostatic adsorption. The behavior of the capsules was investigated at an air–water interface after their introduction into an aqueous subphase. The interaction of the capsules with 1,2-Dimyristoyl-sn-glycero-3-phosphocholine (DMPC) monolayer formed on the water surface (model cellular membrane) was studied both upon their introduction under the condensed monolayer and with the use of a dilute colloidal solution of the capsules as a subphase. Biodegradation of the proteinaceous capsules with subsequent oil-core release was demonstrated by influence of pronase. The Langmuir lipid monolayer was found to be a good model for investigation of drug release from the capsules in the presence of the cellular membrane.
