The main biological properties of probiotic bacteria are presented in a context of their modulating activity on an inflammatory immune response. Available data on the reduction of a possible risk, duration, and severity of respiratory infection symptoms during a probiotic medication intake were analyzed. Potential antiviral probiotic mechanisms have been reviewed and discussed.

The literature data on the study of the microbiota of female genital organs and assess the clinical and microbiological characteristics of the probiotic strain LCR35, used in the treatment and prevention of vaginal infections were analyzed. Biotherapeutic aspects of treatment and prevention of dysbiotic conditions of vaginal microbiota have been analyzed. A comparative evaluation of the characteristics and requirements of lactobacillicontaining preparations with probiotic activity in obstetric-gynecological practice was carried out. The clinical and microbiological advantages of the original probiotic strain Lactobacillus casei rhamnosus Doderleini (LCR35) in the formulation Lactogynal in the treatment and prevention of vaginal infections were noted. The literary review of data the medical use of the culture of Lactobacillus casei rhamnosus Doderleini (LCR35) testifies to the expediency and validity of its use in the mode of two-stage therapy of the dysbiotic conditions of the vagina. Organization and conduct of additional post-registration clinical studies of medicinal product for medical use Lactogynal will strengthen the position of targeted probiotics in the complex treatment and prevention of bacterial and fungal infections of female genital organs.

In order to prevent the phenomena of "microbiological genocide", unjustified and unfavorable violations in microecological systems, and, first of all, human microbiota during the implementation of the disinfection prevention of diseases, the necessity for the scientific justification and practical provision of sufficient antimicrobial selectivity of disinfecting effects, is substantiated in the article. It is necessary to decode and take into account the susceptibility of different microbe pathogens to various disinfectants in comparison with saprophyte, and even more so, useful microflora.

To carry out a systematic analysis of the data available in the literature on rational therapy for vaginal microbiocenotic disorders during combined antibacterial therapy to optimize the use of probiotics. Material and methods. The review includes the data of foreign and Russian articles found in the international system Pubmed. Results. The polymicrobial etiology of the inflammatory process requires combined antibacterial therapy using 2–3 antibiotics, which predisposes to dysbiotic states and the growth of opportunistic pathogens, including Candida fungi. It is advisable to prophylactically take probiotics simultaneously with the initiation of antibiotic therapy, which is more effective than the use of probiotics immediately after completion of an antibiotic therapy cycle. The vagina can be colonized by probiotic bacteria following oral administration if acid-resistant strains of lactobacilli are used. Conclusion. It is expedient to simultaneously take acid-resistant probiotic strains per os during antibiotic therapy in order to restore a normal protective flora and to suppress the opportunistic intestinal microorganisms that are able to colonize the urogenital area.

The review gives the definition and conditions of assigning a biological agent to probiotics, the mechanisms of action of probiotics and microbiota on the activity of the central nervous system, as well as the main biologically active amines synthesized by the microbiota. It considers the neuroendocrine effects of probiotics and microbiota and their role in the pathogenesis of obesity and diabetes. The authors summarize the studies that prove the impact of the microbiota on metabolic processes via synthesis of vitamins B and D and the ensuing prospects for their therapeutic application.

The review presents data on the relationship of intestinal dysbiosis with irritable bowel syndrome (IBS) and the possibility of modulation of the intestinal microbiota (MB) in IBS therapy. It is shown that imbalance of MB caused by the influence of diet, antibiotics or other exogenous factors leads to gastrointestinal disorders, in particular, to the development of IBS. The data are presented that the modulation of MB with the help of prebiotics, antibiotics, probiotics and fecal microbiota transplantation has a positive therapeutic effect in patients with IBS.

В статье представлен аналитический обзор научных статей, опубликованных в электронных библиотеках Cochrane Library, MEDLINE, PubMed, E-library за последние 15-20 лет и посвященных изучению проблем микробиоценоза кишечника, начиная от общих понятий данного состояния до современных взглядов на терапию нарушений микробного состава тонкого и толстого кишечника. Авторы прослеживают эволюцию лечебных подходов от пробиотиков, пребиотиков, симбиотиков до метаболической терапии. В обзоре анализируются преимущества и недостатки пробиотической терапии, обсуждаются предлагаемые пути решения возникших проблем, таких как риск трансгенной передачи информации между бактериями и эндотелиальными клетками. В современной гастроэнтерологии разрабатываются новые подходы к лечению дисбиоза кишечника. Изучение метаболома эукариотов помогает синтезировать новые препараты - метаболики. Метабиотики, в качестве регуляторов физиологических функций, биохимических и поведенческих реакций, имеют ряд преимуществ по сравнению с пробиотиками и пребиотиками, в связи с благоприятным профилем безопасности и длительным сроком хранения, обладают лучшей абсорбцией, легко распределяются и выводятся из организма. Метабиотики являются эволюцией развития пробиотической концепции. Одним из представителей метаболических препаратов является Хилак форте. Эта свободная от бактерий жидкость содержит метаболиты бактерий Escherichia coli DSM 4087 (25 g), Streptococcus faecalis DSM 4086 (12.5 g), Lactobacillus acidophilus DSM 4149 (12.5 g), и L. helveticus DSM 4183 (50 g). Хилак форте имеет долгую историю применения и может быть применен на любом этапе коррекции дисбиотических нарушений широкой категории пациентов.
